The first form of Sailor Moon is the longest lasting. This is the only form to have three separate incarnations, Prism, Crystal, and Cosmic. Usagi defeated more enemies than any other in this form. It is also considered to be the iconic Sailor Moon form, since it is the original.
Despite the fact that this form is the weakest, it had the biggest variety of attacks. Also, it utilized more special items than any other forms combined. Sailor Moon draws her power from various sources. The first, Prism form, is unknown, but the Crystal form is powered by the Ginzuishou. Sailor Moon's Cosmic form was created from Prince Endymion and Princess Serenity's love.
Sailor Moon is not the leader of the Sailor Team, but she is a pivitol member since her attacks are the strongest and she is almost always the one responsible for destroying the team's opponent at the end of battle. Since she is also the reincarted soldier form of Princess Serenity, she is the one her teammates are sworn to protect.
Costume:
The Prism and Crystal forms have identical costumes besides for the change of the brooch to the compact. The Cosmic form has a few new additions. Here is the breakdown.
The Sailor Suit has a blue collar with two white stripes and a matching blue pleated skirt. The bodice is white, and the sleeves are three sections of the same white fabric. A white "protector" covers the entire upper chest and upper back area. A white, v-shaped belt connects the bodice to the skirt. A red bow attaches to the front and center of the chest, and the brooch/compact is affixed on the midpoint. A matching red bow is on the center of the lower back, just above the start of the skirt. Sailor Moon's gloves are elbow length, the top decorated by three red cylindrical sections. And finally, her red boots are vinyl or patent leather with a white trim on the top and a golden moon is on the front.
As for accessories, Sailor Moon wears two red, white-trimmed "covers" over her odango. Her standard tiara is gold with a red jewel, and she wears earrings; tiny gold moons that dangle from three pearls. The choker is red with a gold moon in the center.
The Cosmic form is exactly the same except the red jewel on the tiara has been replaced by a yellow moon and the gold moon on the choker has been replaced with a yellow heart.

This form has more items than any other, with a total of 13 items.

Usagi receives the Henshin (Transformation) Brooch from Luna in episode 1. It is used to transform her into Sailor Moon using "Moon Prism Power Make Up!" More

The Silver Millennium relic belonging to the Moon's Royal family.Given to Usagi in episode 25 by Luna. It is used to perform "Moon Healing Escalation." More

Orgel:
Tuxedo Mask drops the Orgel after a battle with the Dark Kingdom, and Sailor Moon keeps it. It would later be used by Sailor Moon to help a possessed Prince Endymion remember his true self.

Maboroshi no Ginzuishou/Illusionary Silver Crystal:
The ginzuishou is first seen in episode 34 and is used by Sailor Moon to transform using the phrases "Moon Crystal Power, Make Up!" and "Moon Cosmic Power, Make Up!"

Crystal Star:
Usagi's second transformation tool, given to her by Queen Serenity in episode 51. Is used with the phrase, "Moon Crystal Power, Make Up!"

Cutie Moon Rod:
Sailor Moon's primary weapon from the R series, given to her by Queen Serenity in episode 51. "Moon Princess Halation" is its only known ability.

The third transformation tool, born from Serenity and Endymion's love in episode 91. Usagi transforms into Sailor Moon by saying "Moon Cosmic Power, Make Up!"

Spiral Heart Moon Rod:
The weapon born from Serenity and Endymion's love in episode 91. It is used by Sailor Moon's Cosmic form to perform the attack, "Moon Spiral Heart Attack."

The Holy Grail is formed when the three Talismans come together in episode 111. Sailor Moon uses it to temporarily transform into Super Sailor Moon, and keeps it in her possession. More

Moon Prism Power, Make Up:
Usagi uses the Henshin Brooch to transform into Sailor Moon. This first appeared in episode 1, and is last used in episode 51. It was her first and only non-compact brooch.

Usagi uses the Crystal Star to transform. The Crystal Star was a gift from Queen Serenity, and was first used in episode 51. It is also the first compact to contain the Ginzuishou inside.

Usagi uses the Cosmic Heart Compact to transform. The Cosmic Heart Compact is an upgrade of the Crystal Star, and was first used in episode 91.

Magic Powers and Physical Attacks:
Sailor Moon has a wide variety of attacks and healing abilities.

Supersonic Waves:
This is an involuntary reaction when Sailor Moon panics and has no other way to protect herself. Her odango covers amplify her wailing, causing pain for her opponents. It appears in episode 1.

Moon Tiara Action!
Sailor Moon uses this attack to defeat youma in the early part of the first series. She changes her tiara into a disc of energy and throws it at her enemy. It is first used in episode 1.

Moon Power, ____ Ni Nare! Moon Power, Transform Me Into a ____!
Usagi uses the Light na Hensou Pen with this phrase to transform herself into whomever she wants. It was first used in episode 3. It was only used in the first two series.

Moon Tiara Stardust!
In episode 5, an alternate version of Moon Tiara Action was used, Moon Tiara Stardust. This is Sailor Moon's first purification maneuver. This was the only time it was ever used.

Sailor Moon Kick!
This is an offensive attack Sailor Moon uses in episode 17 and episode 19. She simply kicks her opponent.

Moon Healing Escalation!
Sailor Moon uses this ability to heal the Seven Youma. It is performed using the Moon Stick, and first appears in episode 25.

Moon Princess Halation!
Sailor Moon uses this as a general offensive maneuver throughout the Sailor Moon R series, and it first appears in episode 51. It is performed with the Cutie Moon Rod.

Sailor Body Attack!
This is another physical attack Sailor Moon only uses in episode 61. She throws her body at her opponent.

Moon Spiral Heart Attack!
Sailor Moon uses this as her primary attack in the Sailor Moon S series. The Spiral Moon Rod is born from her and Mamoru's love for one another. This attack was first used in episode 91.

Sailor Teleport!
Sailor Moon, along with the other Inner Soldiers use this ability to teleport themselves to any desired destination. It is first used in episode 45.

Sailor Planet Power!
This is an attack used by the five Inner Soldiers to shoot a beam of energy straight up into the air. It can be performed without Sailor Moon, but it is half as powerful. First used episode 72.

Sailor Planet Attack!
This is very similar to Sailor Planet Power, but it can be performed only if all five Inner Soldiers are present. This attack sends a beam of energy at a desired target. First used episode 82.

Defeats and Rebirths
Sailor Moon has been defeated several times, but a new power is always found so she can continue to fight.

End of Prism:
At the start of Sailor Moon R, Usagi has lost her will to fight, and her brooch is destroyed easily by the Cardian Reci in episode 51.

Birth of Crystal:
Queen Serenity intervenes as the Sailor Team is losing a battle and Sailor Moon is unable to transform. She upgrades the brooch to the Crystal Star in episode 51.

End of Crystal:
Sailor Moon's compact is viciously bitten by the Daimon Mikuuji, causing the light of the Ginzuishou to go out. Sailor Moon detransforms instantly and is unable to transform again.

Birth of Cosmic:
Created by the love of Prince Endymion and Princess Serenity, a wave of pink energy explodes between them, morphing the Crystal Star into the Cosmic Heart Compact.

 |
End of Cosmic:
Cosmic is never used again after Pegasus permanently upgrades the Cosmic Heart Compact into the Crisis Heart Compact, making Sailor Moon an obsolete form, in episode 130.
